1. Each club may have up to two adults (does not need to be an advisor) and one youth.
2. Clubs may not change their representatives during the year. In other words, the names on the forms are those who will represent your club for 2019.
3. Only those persons whose names are on the forms may vote at committee meetings.
4. If the forms are not returned, your club will not have voting privileges at committee meetings. You may NOT simply show up at a committee meeting and say you are the representative(s) for a given club.
The form must be filled out by an advisor, submitted, and received by February 8, 2019. Alternatively, you may simply email the information to firstname.lastname@example.org
